Description

The functional pathologies of the abdominopelvic sphere out of which urinary incontinence is the most frequent, affect numerous persons, without distinction of age or sex. For most, this ailment represents a major handicap which reduces significantly the quality of life.

Moreover, a study realised in 1993 has allowed to demonstrate on the basis of a control group, that incontinent persons present a greater predisposition to sleep disorders, accompanied by a reduction of vitality as well as mobility. Other symptoms such as social isolation and sometimes emotional disorders go along with these dysfunctions.

Urinary incontinence counts amongst the most frequent functional disorders in the population. To this extent, it constitutes a real issue of public health where physiotherapists have an important role to play.

Objectives and skills developed

This seminar has as objective to reinforce the knowledge of anatomy, of physiology and physiopathology of functional disorders in the abdominopelvic sphere. It sketches the principles and practical application of the treatment of these disorders in physiotherapy.

At the end of this introductory seminar, the participants will have acquired the analytical elements of functional pathologies which affect the balance of the lesser pelvis. They will be able to identify the urinary and fecal symptoms linked to these pathologies, find the warning signs of functional disorders, act preventively and elaborate a global rehabilitation approach.

Contents and specificities

The abdominopelvic care by physiotherapists falls within what is called the concept of perineology, which deals with the anatomic and functional balances of the various abdominopelvic structures. The various organs of the lesser pelvis such as the bladder, the uterus and the rectum…must be considered within the reasoning of the perineal physiology. They are interrelated both on an anatomic as well as on a functional level.

The physiotherapist, in accordance with clinical criteria, can intervene on a functional level, with the aim to reduce disorders of the pelvic statics. These are often the result of a collapse of the support systems of the organs.

If urinary incontinence linked to pregnancies and deliveries is now well-known by the general public, others such as the urinary incontinence of female athletes, the urinary incontinence linked to stress, to emergency or overflow, static disorders leading to prolapsed or syndromes of the descending perineum….remain largely unknown and are sometimes even taboo.

For men, post-prostatectomy urinary incontinence when arising is still too often considered as a fatality although a rehabilitation by a professional can significantly improve this functional disorder and in some situations, even restitute an efficient continence.

Perineal physiotherapy allows also to assist patients who suffer from ano-rectal pathologies such as colic and terminal constipation, anal incontinence…

For children, immaturity of the bladder, encopresis, as well as enuresis can also favorably benefit from functional rehabilitation.
